Driving Anxiety Screen

This assessment is designed to help us (and you) learn about your driving anxiety and how it might be affecting you, how you think and feel, your life roles, and your level of independence. Please note that while some questions help us understand how you are feeling, some are merely demographic in nature to help us understand our client population.

If you choose to participate in one of our programs, your responses will also serve to provide a baseline of where you started at the beginning of the program. You will be encouraged to take the same questionnaire at the end of the program to see the changes that have occurred. If you do not participate in a program offering, we hope that it will still give you valuable information that you can use to better understand your driving anxiety.
